For established patients, once your appointment is scheduled, kindly complete the necessary forms sent to your email before your visit.


DO NOT BOOK A NEW PATIENT or HOSPITAL FOLLOW-UP APPOINTMENT ONLINE - Please call the office directly. 


***IMPORTANT INFORMATION***

If you are experiencing urgent symptoms, please call our office, or seek immediate medical attention by calling 911 or visiting your nearest emergency/urgent care. Please do not book online for an urgent appointment. 


Medicaid/Workman's Comp

Unfortunately, we are not contracted with AHCCCS, or any state Medicaid at this time. We also are unable to process Workman's Comp claims.  


3% Card Fee

A 3% fee will be added to the total for ALL card transactions (debit, credit, HSA, etc.). This fee can be avoided by paying with cash or check at the time of services rendered.
