For established patients, once your appointment is scheduled, kindly complete the necessary forms sent to your email prior to your visit.
***IMPORTANT INFORMATION*** If you are experiencing severe symptoms such as shortness of breath, difficulty breathing, chest pain, high fever, or any other urgent health concerns, please DO NOT SCHEDULE AN ONLINE APPOINTMENT. Instead, seek immediate medical attention by calling 911 or visiting your nearest emergency room or urgent care facility. If you have been in the hospital or urgent care please call our office to schedule a follow-up for continued care.
To secure your booking, we require a card on file in accordance with our no-show policy. Rest assured, this card will only be charged if you miss your appointment without notifying us in advance. It will not be stored or used for payment of services. We appreciate your understanding and cooperation. Thank you!
A 3% fee will be added to the total for any card transactions (debit, credit, HSA, etc.). This fee can be avoided by paying with cash or check at time of service.
